 The Long Term Home Health Care Program is available to patients who are chronically ill and need ongoing care. The goal of this program is to maintain the patient at home and prevent institutionalization by providing a comprehensive mix of services.
Under this program, the following services are available to you in addition to those provided by the Certified Home Health Care Program:
- Personal Emergency Response
- Personal Care Aide visits
- Home Delivered Meals
- Housekeeping
- Nutritional Counseling
- Social Day Care
- Transportation to Day Care
